I acknowledge that I am placing a deposit for tattoo services to be rendered at a later date. I am aware that the deposit will be applied to the cost of the tattoo on the date of the appointment. I am also aware that ALL DEPOSITS ARE ENTIRELY NON-REFUNDABLE, (this includes, but not limited to: travel changes/cancellation, hurricane/weather, pandemic, illness, death, alien invasion, locust, astroid). I understand that the deposit will be lost if I no longer want the tattoo, miss the appointment for ANY reason or arrive for the appointment more than 20 minutes late. I am also aware that the deposit will be lost if I fail to give at least 48 hours notice in the event of a desire to reschedule the appointment, or reschedule my appointment more than 1 time.

I agree to provide proof that I am at least 18 years of age in the form of a state or government issued photo id on the date of the appointment.
